Positron emission tomography with macrophage targeting to select individuals at risk for rheumatoid arthritis.

Rheumatoid arthritis is an inflammatory condition that causes painful, swollen joints. Some people experience joint aches and carry specific blood markers called ACPA, which put them at higher risk of developing the condition, but doctors cannot yet tell who will definitely develop it or when. This clinical study tests an advanced imaging technique called a PET scan using a special tracer called [18F]Fluor-PEG-Folate. This tracer targets specific active immune cells linked to joint inflammation. By scanning the joints, researchers hope to see whether early hidden inflammation shows up before swollen joints appear. Participants will have the scan and then be monitored over the following year. If successful, this scanning method could help doctors identify high-risk individuals early, potentially allowing for earlier care and better protection for joint health in the future.

על מה המחקר הזה?

Rheumatoid arthritis is an autoimmune condition where the body's immune system mistakenly attacks the lining of the joints, leading to pain, stiffness, and swelling over time. Many people go through an early stage where they have joint pain and a positive blood test for antibodies known as ACPA, but no visible swelling yet. While this means they are at higher risk, not everyone with these antibodies goes on to develop full rheumatoid arthritis.

At present, it is difficult for doctors to predict exactly which patients will progress to rheumatoid arthritis and how quickly it will happen. This study aims to find out if a specialised body scan can give clearer answers. Researchers are testing a type of PET scan that uses a harmless tracking dye called [18F]Fluor-PEG-Folate. This dye attaches to active immune cells, called macrophages, that gather where inflammation is starting.

By taking clear pictures of the joints with this scan, doctors hope to detect early, hidden inflammation before physical swelling begins. Participants will be followed up for a year to see if those with positive scans are more likely to develop arthritis. Finding a reliable way to spot early changes could help doctors step in much sooner in the future, offering treatments before joints become damaged.

נקודות עיקריות

  • Tests a specialised PET scan to predict who might develop rheumatoid arthritis.
  • Aimed at adults with joint pain and positive ACPA blood test results.
  • Uses an imaging dye designed to highlight early, hidden immune activity in joints.
  • Involves a hospital scan followed by health monitoring over a 12-month period.
  • Taking part is completely voluntary and will not affect your standard medical care.

מי עשוי להיות זכאי?

This study is open to adults aged 18 and older who have persistent joint pain (arthralgia) and have tested positive for a specific antibody in their blood called ACPA. These antibodies are linked to an increased risk of developing rheumatoid arthritis.

To be eligible, you must have joint pain without having developed visible joint swelling or a confirmed diagnosis of full rheumatoid arthritis yet. People of all genders can take part.

Your study doctor will check your medical history to make sure taking part is safe for you. If you already have a diagnosed form of inflammatory arthritis, or if you cannot safely have a PET scan, you may not be able to join.

האם מחקר זה יכול להתאים לך?

ענה על השאלות המהירות הללו כדי לבדוק אם אתה עשוי להיות זכאי. זהו מדריך בלבד — צוות המחקר מקבל את ההחלטה הסופית.

  1. I am aged 18 or older.
  2. I experience ongoing joint pain or discomfort.
  3. I have been told by a doctor that I have positive ACPA antibodies in my blood.
  4. I have not yet been diagnosed with full rheumatoid arthritis or swollen joints.
  5. I am willing to undergo an injection and lie flat for a PET scan.

מה כרוך בהשתתפות?

Taking part in the study involves an initial screening visit where the team will check your joint symptoms, review your blood tests, and confirm that you are eligible.

During the study, you will attend a hospital visit for a PET scan. A small amount of an imaging tracer will be given through a quick injection into a vein in your arm. You will then lie comfortably inside the scanner while detailed pictures of your joints are taken.

After the scan, the study team will monitor your joint health over a period of roughly one year. You will have follow-up visits or check-ins so doctors can see whether you remain well or if you develop any joint swelling. In total, your participation will last about 12 months.

סיכונים ותועלות פוטנציאליים

Taking part may not directly improve your joint pain, but it provides regular monitoring by joint specialists and helps advance research that could protect others in the future. The scan involves a small amount of radiation from the imaging tracer, similar to other standard medical scans. You might also feel brief discomfort or bruising from the injection. Serious allergic reactions to the tracer are very rare. Taking part is entirely voluntary, and you can change your mind and withdraw from the study at any time without your regular NHS care being affected.

שאלות נפוצות

What is an ACPA blood test?

ACPA stands for anti-citrullinated protein antibodies. It is a routine blood test doctors use to check for immune markers commonly linked with rheumatoid arthritis.

Does having a positive scan mean I definitely have arthritis?

No. The scan is designed to show very early signs of immune activity. The purpose of this trial is to learn whether that activity reliably predicts future joint swelling.

Is the PET scan painful?

No, the scan itself is completely painless. You will only feel a quick pinch when the imaging dye is injected into your vein, and you will need to lie still while pictures are taken.

Will taking part change my usual treatments?

The study focuses on imaging and observation. Your medical team will continue to advise you on standard pain relief and care for your joints throughout the study.

What happens if my joints become swollen during the year?

If you develop swollen joints, the study doctors will assess you promptly and guide you on the best standard treatment options available.
